The City of Stamford Department of Health & Human Services is committed to transparency and public access to government records. Under the Connecticut Freedom of Information Act (FOIA), members of the public have the right to request access to public records maintained by the Department of Health & Human Services, subject to certain legal exemptions. 

Submit a Request for General Plan Information 

You may request existing records maintained by the Stamford Department of Health & Human Services, Environmental Health & Inspections Division, including: 

  • Septic System Plans 
  • Well Completion Reports 
  • Environmental maps and other related records.

Requests are reviewed and processed within 3 - 5 business days from your submission. You will receive notification confirming receipt of your request and provided a Record ID #. Should you not receive this confirmation within 5 business days, please email the Environmental Health Leadership Team at StamfordDIRHLTH@StamfordCT.gov

Submit a Freedom of Information (FOI) Request 

You may request existing, non-exempt records created or maintained by the Stamford Department of Health & Human Services, including: 

  • Public Health Codes, Policies, and Regulations 
  • Environmental Health Inspection Reports 
  • Licenses and Permits issued by the Department 
  • Aggregate Public Health Data & Statistics 
  • Correspondences and Administrative Records related to the Department's operations.
